Marriage License Application

By law, you must apply for a license in the Connecticut town where you plan to be married. The license will be valid for 65 days from the date of issue. An expired license will become void and must be reapplied for. The completed license must be returned to the town you were married in.  Only the officiator may return the license. Please have ready the contact information of your officiator.

MARRIAGE LICENSES WILL ONLY BE ISSUED BY APPOINTMENT.  PLEASE CALL AHEAD TO SCHEDULE A TIME.  860-767-4340, EXT 128 or 129.

Both parties must appear at the Town Clerk's Office in person where you will present the required ID, fill out the form, talke the oath, and sign the license.

The application form will be provided for and must be filled out in the presence of the town clerk. You will questioned under oath whether or not the information that you have provided is true and correct. The license fee is $50.00. Certified copies will cost an additional $20.00 each.

1. IDENTITY:

You must present two (2) forms of identification; at least one of the documents must be from the Primary document list.

PRIMARY document list

US born

  • US Passport or Passport Card
  • US photo driver license (in or out of state)
  • US Territory or Canadian photo driver license

Non-US born

  • US Passport or Passport Card
  •  Valid Foreign Passport
  • Certificate of Naturalization
  • Certificate of Citizenship
  • Permanent Resident Card
  • US Consular Report of Birth Abroad (Issued by the Department of State)

SECONDARY document list

  • Out of State US photo driver license
  • US Territory or Canadian photo driver license
  • Out of State US or Canadian issued photo learner permit
  • Other foreign driver license
  • Connecticut issued non-driver identification card, driver license or learner permit
    (Your driver license or ID may be required to be surrendered)
  • US Military ID or dependent card with photo
  • Connecticut State Permit to Carry Pistols or Revolvers
  • Military discharge/separation papers (DD-214)
  • Pilot’s license (issued by the US DOT Federal Aviation Administration)
  • Certified school transcript (school photo ID not acceptable)
  • Social Security Card (Not laminated or metal. 16 and older must sign)
  • CT Department of Corrections certificate (CN101503)
  • Baptismal certificate or similar document
  • State or Federal Employee Identification with signature and photo and/or physical description with or without date of birth
  • Employment Authorization card
  • Veteran Health Identification Card (VHIC)
  • Federally Recognized Tribal Member ID card
  • DHS Trusted Traveler Cards (Global Entry, NEXUS, SENTRI, FAST)
  • Stamped visitor’s VISA or (VISA waiver certification, in conjunction with an e-passport - Foreign Applicants)

Additionally, these identification documents must display the full name and current address:

  • Cable or Internet Bill (not older than 90 days, showing your account number, full name, and current address)
  • Water Bill (not older than 90 days, showing your account number, full name, and current address)
  • Phone Bill (not older than 90 days, showing your account number, full name, and current address)
  • Gas Bill (not older than 90 days, showing your account number, full name, and current address)
  • Electric Bill (not older than 90 days, showing your account number, full name, and current address)
  • Bank, Loan, or Financial Institution Statement (not older than 90 days, showing your account number, full name, and current address; issued by the bank)
  • Medical Bill (not older than 90 days, showing your account number, full name, and current address)
  • Pay Stub (not older than 90 days)

 2. SOCIAL SECURITY NUMBER:

Please present one of the following to prove your Social Security number. The 1099 or W-2 must show your full Social Security number and is only for proof of Social Security number, not identity.

  • Social Security card
  • W-2 form (issued within 5 years)
  • 1099 (issued within 5 years)
